-Primary cause: acute hypoxic respiratory failure, PNEUMONIA, COVID-19

-Secondary cause: prostate cancer; closed head injuries due to probable fall

62 year-old black male

Location of death: 6141 N. Pulaski, Chicago

Died from:

-Primary cause: COVID-19

-Secondary cause: coronary artery disease; hypertension; chronic substance abuse

66 year-old black female

Location of death: 11 Choate Road, Park Forest

Died from:
