AMITA ADVENTIST LA GRANGE MEMORIAL HOSPITAL: Health Provides Free Assistance for Healthcare Insurance Enrollment

AMITA Adventist La Grange Memorial Hospital issued the following announcement on Dec. 3.

The COVID-19 pandemic has left many in Chicagoland without the health insurance they need, when they need it most.

AMITA Health has partnered with Advocatia Solutions in providing free help in finding no- or low-cost health insurance options through federal, state and local programs and the Healthcare Insurance Marketplace, no matter where care is provided.

With the open enrollment period is ending on Tuesday, Dec. 15, 2020, individuals and families are encouraged to learn what programs they may be eligible for by logging onto amitahealth.org/enroll or calling 312.584.1212. Information may also be obtained by texting “AMITA” to 312.586.9711.

Our team will identify programs available based your unique situation, walking you through the enrollment process. Programs for possible enrollment include Medicaid, the Children’s Health Insurance Program (CHIP), the Supplemental Nutrition Assistance Program (SNAP) or the Health Insurance Marketplace.

To enroll, you’ll need your state driver’s license or ID, 2019 tax return, Social Security number each the members of the household, pay stubs and employment information for all adults and an email address. The enrollment assistance is free, sponsored by AMITA Health.

In addition, all are invited to participate in our Virtual Enrollment Days, to be held from 8 a.m. to 8 p.m. on Saturday, Dec. 5, and Tuesday, Dec. 8. Call 312.584.1212 for an immediate assessment of eligibility.
