For a growing number of patients, home health is a welcome alternative to care in a hospital or nursing center. Home health can include a variety of services offered in a patient’s home or other place of residence. The care can be provided by nurses, therapists or aides and is tailored to the unique needs of each patient.
Patients with diabetes or diabetic conditions can receive care at home that includes assistance, education and coaching on:
Self-management of diabetes to maintain blood sugar levels and decrease the risk of complications
The progression of your diabetes and how to minimize the risk of organ damage such as heart disease, kidney failure and vascular disease
Blood sugar self-monitoring and target blood sugar ranges
Management of diet, medication and lifestyle to maintain healthy blood sugar levels
Early signs of complications and when to alert your physician about changes in your condition
