City of Northlake issued the following announcement on Mar. 5.
Call the Illinois Department of Health at 1-800-889-3931 to see if you are eligible for the COVID-19 vaccination.
Then call Walgreens at 1-800-925-4733 or go to https://www.walgreens.com/findcare/vaccination/covid-19 to schedule your COVID-19 vaccination.
Limited supplies of COVID-19 vaccines are now available in some states at select stores to individuals 16 years of age and older (16 years of age and older for Pfizer; 18 years of age and older for Moderna) who meet state-specific eligibility criteria. If you don’t meet your state’s eligibility criteria for a COVID-19 vaccination, you are not eligible to receive a vaccination at this time. See if you’re eligible in your state, call the Illinois Dept. of Health at 1-800-889-3931.

Complete a short eligibility screening.
If you’re eligible, we’ll look for available appointments in your area for your COVID-19 vaccination appointment.
Then call Walgreens at 1-800-925-4733 for COVID-19 testing.
