Alden Manor Rehabilitation and Health Care Center, a 249-bed skilled care facility in Cicero, was cited by the Illinois Department of Public Health (IDPH) for violations of the Nursing Home Care Act in the fourth quarter of 2018.
The report lists the nursing homes in the state with A violations, which might have caused death or serious injury to a resident.
Alden Manor Rehab was cited with an A violation for an incident in November 2018 and fined $25,000.
