1) Approval of Village of Maywood Warrant List No. 200451 through April 26, 2018 in the amount of $1,228,836.84.

2) Discussion concerning the Village of Maywood Open Invoices Report as of April 26, 2018 in the total amount of $2,654,546.26.

B. Mayor's Report and Theme: "A Shift Is Coming, Things Are Changing In Our Favor, When The People Get A Mind To Work"

1) Presentation by Mr. Alfred Murray of Edelson PC concerning Opioid. This item was presented at the February 20, 2018 Regular Board of Trustees Meeting.

9. Village Manager's Report Agenda Item(S):

A. Presentation by CS Insurance Strategies concerning the Medical Insurance Renewal Rates for 2018-2019 and updated plans to implement online enrollment for employees in the Village of Maywood. (No attachments)

B. Discussion and consideration regarding engineering agreement pursuant to the Preliminary Engineering for the First Avenue Water Main Project from Harrison Street to Washington Boulevard with a memo dated April 24, 2018 from Hancock Engineering Co. The Project costs and related professional fee expenses are payable from the Madison Street / Fifth Avenue TIF District.

C. Discussion and consideration regarding engineering services pursuant to the 2018 Wilcox Avenue Improvement Project. Wilcox Street is the placeholder for the right-of-way created behind National Cycle and Supreme Catering from 19th Avenue to 21st Avenue in the Village of Maywood along with a cover memo from Hancock Engineering dated April 24, 2018. The Project costs and related professional fee expenses are payable from the Madison Street / Fifth Avenue TIF District.

D. Discussion and approval of Installing Electrical Cabinet and Connection in Winfield Scott Park in the Village of Maywood. The funding will come from the Madison Street / 5th Avenue TIF. Requesting waving the bid requirement because H&H is the Village of Maywood current Electrical Contractor.

E. Discussion of the proposed cornice and masonry repairs for the Maywood Masonic Temple, Multipurpose Building located at 200 S. 5th Avenue.

F. Maywood Park District is requesting to provide a presentation from KaBoom to the Village of Maywood to give permission to place playground equipment at Winfield Scott Park through a Grant Program.

G. Discussion and approval to enter into a cleaning and building maintenance program with City Wide for a cost of $7,200.50 per month. This project will be for the Village of Maywood Village Hall(40 Madison Street) Police Department(125 So. 5th Ave.) and Multipurpose Building (200 So. 5th Ave.)

H. Discussion to select an asbestos abatement contractor to remove asbestos transit siding located at 1304 S. 21st Avenue, (FastTrack Demolition Project Round (1).

A. Status Memo regarding Chronic Public Nuisance at 5th Avenue/Quincy Street Apartment Building, dated April 25, 2018 from Klein, Thorpe and Jenkins, Ltd.

An Ordinance Amending Title Iii (Administration) Of The Maywood Village Code To Add Chapter 40 (Pseba Benefit Administration) With Regard To Administrative Procedures For Assessing And Determining Claims Under The Public Safety Employee Benefits Act, with a cover memo dated April 25, 2018 from Klein, Thorpe and Jenkins, Ltd.

A. Motion to approve the travel reimbursement expense report of Trustee Kimyada Wellington for the 2018 National League of Cities Conference in the amount of $122.99.

Resolution Approving A Depositary Agreement Between The Village Of Maywood And Amalgamated Bank Of Chicago, As Depositary For The Establishment And Maintenance Of An Account Containing The Village Of Maywood Working Capital Reserve Fund (the Agreement is attached as Exhibit “A” to the Resolution), with a cover memo dated April 25, 2018 from Klein, Thorpe and Jenkins, Ltd.

F. Approval of payment to Blue Cross Blue Shield for Health Insurance Premiums for the month of May 2018 in the amount of $289,791.16.

H. Approval of payment to H&H Electric Company for street light maintenance and repair for the Village of Maywood Public Works Department in the amount of $6,792.80.

I. Approval of payment to Twin Supplies, Ltd. of Oak Brook, Illinois for Street Lighting Projects, etc. for the Village of Maywood Public Works Department in the amount of $262,035.83.

J. Approval of payment to City of Chicago - Department of Water for water services provided to the Village of Maywood from February 15, 2018 through March 21, 2018 by the City of Chicago in the amount of $317,436.24.

K. Approval of payment to Fleet Services for Retail Fuel Purchases for the Village of Maywood in the amount of $14,773.83.

L. Approval of payment to Village of Melrose Park for water services provided from the Village of Melrose Park to the Village of Maywood in the amount of $65,249.86.

M. Approval of payment to Seal Guard Inc. for labor and materials to complete all masonry repairs as per proposal dated April 5, 2018 in the amount of $7,820.00.

N. Approval of payment to Knickerbocker Roofing and Paving Company for removal of soffit from two designated areas, engineer inspection, and tear down the remainder of the two metal structures located at 200 South 5th Avenue (Masonic Lodge), Maywood, IL in the amount of $10,750.00.

O. Approval of payment to West Cook YMCA for three installment fees for pool start-up and operations cost at the Fred Hampton Aquatic Center in the amount of $60,000.00.
