A. Finance Management Report(s):

1) Presentation and review of Fiscal Year 2017 Audit Report by Mr. Jason Coyle representing Baker Tilly Virchow Krause, LLP.

2) Approval of Village of Maywood Warrant List No. 200450 through April 12, 2018 in the total amount of $962,057.21.

3) Discussion of Village of Maywood Open Invoices Report as of April 12, 2018 in the total amount of $1,907,065.78.

B. Mayor's Report and Theme: "A Shift Is Coming, Things Are Changing In Our Favor, When The People Get A Mind To Work"

1) Continual discussion concerning D-Erania Stampley, Jazz Desires Corporation, a non-for- profit organization requesting donation from the Village of Maywood for a Jazz Bus instead of the Jazz Trolley to support the upcoming West Suburban Jazz Trolley Tour.

2) Discussion concerning Mr. Eddie L. Williams, patient engaged in care at the Captain James A. Lovell Federal Health Care Center, is requesting to have his Emotional Support Dog, TJ, with him at all times due to his symptoms associated with anxiety and depression.

A. Memo regarding Chronic Public Nuisance at 5th Avenue/Quincy Street Apartment Building and Recommended Actions by Village, dated April 11, 2018 from Klein, Thorpe and Jenkins, Ltd.

B. Memo regarding Implementation of Senate Bill 1451 – Small Wireless Facilities Deployment Act, dated April 11, 2018 from Klein, Thorpe and Jenkins, Ltd.

11. Omnibus Agenda Items:

A. Approval of the renewal of the Village’s Insurance Coverages for the Policy Period of May 1, 2018 through May 1, 2019, as presented in a Proposal by Alliant/Mesirow and CS Strategies in the amount of $706,570.00.

B. Approve of the expenditure of an amount not to exceed $17,500.00 for the Village Manager to hire a brick masonry contractor to perform emergency repairs to the brick façade of the exterior of the 500 South Fifth Avenue Building and to waive the bid process regarding this contract due to the emergency nature of the work and the concern for public safety.

C. An Ordinance Authorizing Certain Expenditures From The Madison Street / 5th Avenue Tax Increment Financing Redevelopment Project Area Fund To Pay For The Televising And Cleaning Of Certain Sewers (Project: Televising and Cleaning of Certain Sewers as part of 2018 Alley and Roadway Improvements Project, 20th Avenue and Warren Street CDBG Improvements Project, and Madison Street LAFO Resurfacing Project), with a cover memo dated April 11, 2018 from Klein, Thorpe and Jenkins, Ltd.

D. An Ordinance Authorizing Certain Expenditures From The Madison Street / 5th Avenue Tax Increment Financing Redevelopment Project Area Fund To Pay For The Maywood Pump Station- Scada Replacement Project And Award Of Contract To Low Bidder – Automatic Control Services (Project: Installation of a new Supervisory Control and Acquisition System (SCADA) to control the Village’s existing water system controlling and reporting on the status of the Village’s water pumping station, elevated water tank, and collecting data from the Ninth Avenue meter vault) (Low Bidder Contract Price: Automatic Control Services submitted a Not to Exceed Price of $28,890.00), with a cover memo dated April 11, 2018 from Klein, Thorpe and Jenkins, Ltd.

E. Resolution Authorizing The Expenditure Of Madison Street / 5th Avenue Tax Increment Financing District Funds To Make The Final Contractor Payment To Close Out The 2017 Roadway Improvements Project And Approving Change Order No. 1 For The Project (Low Bidder Contractor: M&J Asphalt Paving Company Inc.) (with Change Order No. 1 attached as Exhibit “A” and final Invoice No. 2017-0432 dated February 27, 2018 attached as Exhibit “B”) in an amount equal to $210,977.16, with a cover memo dated April 11, 2018 from Klein, Thorpe and Jenkins, Ltd.

F. A Collective Bargaining Agreement between Village of Maywood and Metropolitan Alliance of Police Sergeants – Chapter #486 – May 1, 2014 to April 30, 2019, with a cover memo dated April 11, 2018 from Klein, Thorpe and Jenkins, Ltd.

G. Approval of payment to Accu-Tron Computer Service Inc. for providing on-site technical support and consulting services for the Village of Maywood for the month of April, 2018 in the amount of $6,300.00.

H. Approval of payment to Allied Waste Service for Garbage Roll Over Services for the month of December 2017 and February 2018 in the Village of Maywood in the total amount of $24,826.52.

I. Approval of payment to Allied Waste Service for garbage pick-up services in the Village of Maywood for the month of March 2018 in the total amount of $141,750.00.

J. Approval of payment to Amalgamated Bank of Chicago for Village of Melrose Park Series 1998A Bond due for May 2018 in the total amount of $38,409.75.

K. Approval of payment to CDW Government for the purchase of two CISCO DIRECT w/1 YR SNET 8X5XNBD in the total amount of $6,282.22.

L. Approval of payment to Comcast for recurring Telephone/Ethernet Services at the Maywood Fire Department, Police Station, Multipurpose Building (200 Bldg.) and Village Hall in the total amount of $21,125.30.

M. Approval of payment to Core and Main for the purchase of water and sewer supplies for the Village of Maywood Public Works Department in the total amount of $5,940.00.

N. Approval of payment to Illinois Council of Police and Sheriffs for Health and Dental Insurance Premiums for the month of April 2018 in the total amount of $85,810.65.

O. Approval of payment to Maywood Firefighters Foreign Tax Fund from Illinois Municipal League for payment of the 2% Fire Department Tax on Foreign Fire Insurance Companies for tax year ended June 30, 2017 in the total amount of $30,455.91.

P. Approval of payment to Mercury Systems Corporation for maintenance of Moducom E911 Communications Console/Ultracom E911 telephone system effective 5/1/18 - 4/30/19 for the Village of Maywood Police Department in the total amount of $20,580.00.

Q. Approval of payment to Utility Service Company for 500,000 Pedisphere St. Charles Road TANK-Quarterly for the Village of Maywood Public Works Department in the amount of $6,042.50.
