A Resolution Approving And Authorizing The Execution Of Change Order Number 2 To A Construction Contract For Certain Additional Alley Improvements Relative To The 2013 Alley Improvement Project Located Within The St. Charles Road Tax Increment Financing Redevelopment Project Area (Reconstruction Of Public Alley Improvements: Alley #136 (Between 4th And 5th Avenues, Main Street To Lake Street), Alley #132 (Between 8th And 9th Avenues, Main Street To Lake Street), Alley #135 (Between 5th And 6th Avenues, Main Street To Lake Street)) (Escrow Number 4: Village Projects) (Soil Testing and Removal Costs for Alley #132: Lake Street to Main Street, Between 8th Avenue and 9th Avenue), from Klein, Thorpe and Jenkins, Ltd. C. An Ordinance Authorizing Certain Expenditures From The Madison Street/5th Avenue Tax Increment Financing Redevelopment Project Area Fund To Pay For Certain Meter And Backflow Improvements. (Project: Installation of Meter and Backflow Improvements at the 1st Avenue ComEd Meter/Watermain Service Connection to the Distribution Systems for Fire and Domestic Water. Proposal received from Unique Plumbing for the Installation of new meters and check valves at the ComEd facility for the Village of Maywood. The projected total for this project is $49,312.50 with a waiver of the bid process.

D. Ordinance Amending Schedule Vii (Handicapped Parking Spots And Zones) Chapter 76 (Parking Schedules) Of The Maywood Village Code Relative To Designated Handicapped Parking Spaces And Directing The Placement Of Motor Vehicle Parking Regulation Signs (Designated Handicapped Parking Area: East and West Sides of Public Right-of-Way Adjacent to and Opposite of the 1008 South 17th Avenue Property). Note: This agenda item was discussed at LLOC Meeting Wednesday, May 10, 2017.

E. Approval concerning IPFS Corporation of the 28% down payment for General Liability Insurance Premium Policy for auto, property, crime, excess umbrella and general, etc. for the Village of Maywood. F. Approval of an Ordinance amending Title VII (Traffic Code), Chapter 72 (Stopping, Standing and Parking), Section 72.27 (Overnight Parking Prohibited) of the Maywood Village Code relative to the parking of vehicles. Note: This agenda item was discussed at LLOC Meeting Wednesday, May 10, 2017.

I. Approval of payment to Allied Waste Service for garbage pick-up services for the month of April 2017 in the amount of $141,750.00.

J. Approval of payment to HD Supply Waterworks for the purchase of water and sewer supplies in the Village of Maywood in the amount of $5,966.08.

K. Approval of payment to EmergiTech for the Quarterly Maintenance and Support Agreement for March 1, 2017 through May 31, 2017 in the amount of $7,353.33.

L. Approval of payment to Standard Equipment Company for street sweeper repairs for the Village

of Maywood Public Works Department in the amount of $21,266.78.

M. Approval of payment to Utility Service Company for TANK-Quarterly for 500,000 Pedisphere St. Charles Road for the Village of Maywood in the amount of $6,042.50.

N. Approval of payment to Winkler's Tree Service for tree removals/trimmings for the Village of Maywood Public Works Department I the amount of $18,262.08.

O. An Ordinance Amending Title Xi (Business Regulations) Of The Maywood Village Code To Add A New Chapter 127 (Conflicts With Certain Home Rule County Ordinances: Minimum Wage Law And Sick Leave Law) With Regard To Conflicts With Certain Home Rule County Ordinances (Cook County Minimum Wage And Paid Sick Leave Ordinances).

P. An Ordinance Authorizing Certain Expenditures From Chicago Title Insurance Company Escrow Number 4 (Escrow Account Number 201454332) Established For The St. Charles Road Tax Increment Financing Redevelopment Project Area Fund To Pay For The Village’s Alley Improvement Project (Escrow Number 4: Village Projects) (Soil Testing and Removal Costs for Alley #132: Lake Street to Main Street, Between 8th Avenue and 9th Avenue).

Q. Approval of payment to Accu-Tron Computer Service for computer consulting services for the month of May 2017 in the amount of $6,300.00.
