B. Approval of a Resolution accepting a Grant from the Illinois Housing Development Authorities Abandoned Residential Property Municipal Relief Program. The Grant amount is $75,000.00. Note: Agenda item was discussed at LLOC Meeting Wednesday, April 26, 2017.

C. Approval for the 2017 Roadway Improvements Project - Bid Opening Results and Recommendation. The lowest responsive bidder for the project was M&J Asphalt Paving Co. Inc. of Cicero, IL totaling $979,569.50. Note: Agenda item was discussed at LLOC Meeting Wednesday, April 26, 2017.

D. Approval for Lauterbach and Amen, LLP to prepare the Actuarial Report for the Village and the Village's Police Pension for fiscal years ending April 30, 2017 through April 30, 2018. The cost for April 30, 2017 is $8,650 and the cost for April 30, 2018 is $4,700. Note: Agenda item was discussed at LLOC Meeting Wednesday, April 26, 2017.

Approval of an Ordinance Creating a New Chapter 126 (Transportation Network Companies) of Title XI ("Business Regulations") of the Maywood Village Code relative to the Operation of Transportation Network Companies within the Village. I. Approval of payment to Amalgamated Bank of Chicago of the Village of Melrose Park Series 1998A Bond due for May 2017 in the amount of $88,733.21.

J. Approval of payment to AT&T for telephone account #708-343-4244 0596 for the periods of 2/11/17 through 3/10/17 in the amount of $5,683.26.

K. Approval of payment to Blue Cross Blue Shield for Health Insurance Premiums for the month of May 2017 in the amount of $268,056.17.

L. Approval of payment to City of Chicago Department of Water for water services from February 17, 2017 through March 21, 2017 provided by the City of Chicago in the amount of $323,730.52.

M. Approval of payment to Fleet Services for Retail Fuel Purchases for the Village of Maywood in the amount of $13,486.91.

N. Approval of payment to North East Multi-Regional Training Inc. for training period July 1, 2017 through July 1, 2018 in the amount of $6,460.00.

O. Approval of payment to Unique Plumbing Company for the excavating of collapsed sewer and finished backfilling hole for the Village of Maywood Public Works Department in the amount of $13,677.40.

P. Approval of payment to United Septic Inc. for Vector Truck to clean various sewers and inspection of sanitary lines in the Village of Maywood in the amount of $5,600.00.

Q. Approval of payment to Village of Melrose Park for water services provided from the Village of Melrose Park in the amount of $60,500.83.

R. Approval of payment to WebQA Inc. for GovQA Subscription Annual billing June 1, 2017 through May 31, 2018 in the amount of $9,480.00.

S. Approval of payment to YMCA for three (3) installment fees for Pool Start-up and Operations Cost (1st payment) in the amount of $60,000.00.

T. An Ordinance Authorizing and Approving a Partial Corporate Stock Transfer relative to Arts2, Inc. d/b/a Arts Food and Beverage pursuant to Section 117.27(C, J) (Transfer of License) of the Maywood Village Code.

U. An Ordinance Amending Title XI ("Business Regulations"), Chapter 116 ("Tobacco") of the Village of Maywood Code of Ordinances to increase the Minimum Age of Sale, Purchase and Possession of Tobacco Products from Eighteen (18) Years of Age to Twenty-One (21) Years of Age, and regarding Tobacco Licensing and Enforcement.
